Diagnosis and treatment of infertility

Today, it is estimated that about 15% have fertility problems in one or both partners. According to the instructions of the WHO (World Health Organization), if after one year of trying the desired pregnancy did not occur, you should inform your doctor for a consultation and preliminary tests

  • Cervical swabs
  • Induction of ovulation
  • Hormonal status
  • 3D ultrasound of uterus
  • Spermiogram
  • Insemintaion (AIH)
  • Ultrasound 3D folliculometry
  • Hysterosalpingo-foam sonography (HyFoSy)
IVF

Thanks to experience and equipment, we can treat even the most complicated cases of infertility in the polyclinic. Our first goal is to determine why you are not pregnant and then help you find the best and fastest solution to your problem. Every infertility is unique and should be approached that way.

How IVF works?

From the beginning of infertility treatment, it is important to solve the problem in a certain order, preferably diagnose the cause, and then treat it with the optimal method.

On the day of aspiration the partner gives semen which is processed and prepared for one of the IVF procedures. Depending on the finding, fertilization will be attempted by insemination of ova (in case of normozoospermia) or by ICSI (intracytoplasmic sperm injection) method in which sperm is selected, activated and inserted into a ovum previously prepared for this procedure with a glass micropipette.

Male fertility analysis

Infertility in a couple is defined as the inability to achieve conception despite one year of frequent, unprotected intercourse. In approximately 35 percent of couples with infertility, a male factor is identified along with a female factor.

When analyzing male fertility (spermiogram), among other things, we are able to make the following tests, analyzes and procedures.

Spermiogram

Spermiogram is an ejaculate analysis that determines the total number, concentration, mobility and appearance (morphology) of sperm. In the diagnosis of male infertility, this is the basic examination on the basis of which the further course of action in the treatment of the infertility couple is decided.

A method for storing ova and embryos in which the sample is immersed directly in liquid nitrogen at -196 ° C, which leads to the “glazing” of the liquid part of the cells, the so-called vitrification. With the introduction of this cryopreservation method, oocyte survival increased to 90-100%.

From the very beginning of the treatment of male infertility by micromanipulation methods one of the main problems is the choice of sperm. At a magnification of 200 to 400 times, most sperm anomalies that are visible on conventional fixed. The application of this method in the clinic has led to the development of a modified variant of ICSI called IMSI (intracytoplasmic morphologically selected sperm injection) by which we select morphologically best sperm when performing ICSI.

DNA fragmentation is one of the invisible signs of sperm decay. We cannot distinguish such sperm from others until we stain them in a certain way which helps us to diagnose their increased proportion, but after that we cannot use the remaining healthy cells in therapy e.g. by insemination or in IVF procedure. This method increases the success of insemination and the quality of embryos in in vitro fertilization.

By equipping a new laboratory, every detail has been carefully selected to increase the chances of pregnancy. The laboratory is located in the middle of the new space, isolated from all other rooms by air lock, the walls are made of antibacterial panels used in clean room technology, and paints and adhesives used in construction are selected to contain minimal amounts toxic substances. The process of gamete and embryo culture is completely protected from external influences.

Microsurgical testicular sperm extraction (microTESE) is a surgical procedure used to retrieve sperm from the seminiferous tubules of a male’s testes. It’s used for men with non-obstructive azoospermia, which is when a man cannot produce enough sperm to have a detectable amount in his semen — a common cause of male infertility.
